HOSPITAL; AND (2) DEVELOP BID PLANS AND
SPECIFICATIONS FOR THE REPAIR OF THE DISTRICT’S CREOLE HOSPITAL
FACILITIES DAMAGED
BY HURRICANE LAURA,
INCLUDING A MITIGATION PLAN
A. PURPOSE, SCOPE, AND BACKGROUND
This Request for Qualifications (“RFQ”) is in response to the Lower Cameron Hospital Service District’s (“District”) facilities in rural Creole, Louisiana suffering damages caused by Hurricane Laura in August 2020. Your firm has been invited to submit its Qualifications to provide Professional Architectural Services for: (1) the development of bid plans and specifications for the design and construction of a temporary replacement hospital located at the District’s hospital campus in rural Creole, Louisiana; and (2) for bid plans and specifications for the repair of the District’s twenty-five (25) bed hospital facilities damaged by Hurricane Laura (“Project”). All work product arising hereunder for the Project will be owned by the District. The purpose of the Project is the urgent reintegration of and access to hospital services in Cameron Parish. The temporary replacement hospital is a Critical Need Facility and must be designed to be constructed in a manner to qualify for Federal Stafford Act funding. The District is also eligible to seek Stafford Act funding to assist in its efforts to repair the hurricane’s damages to the existing hospital facilities and in the District’s efforts to develop a mitigation plan. The District may also seek funding from other sources, including but not limited to the U.S. Department of Housing and Urban Development (“HUD”) under the Housing and Community Development Act, as amended, or any other HUD program. Therefore when executing the respective AIA contract, the respondent awarded the work for the Project must comply with Stafford Act and Community Development Block Grant (“CDBG”) requirements, including those under the Disaster Recovery program such as the CDBG Compliance Provisions for Professional Service Contracts from the LA OCD-DRU Administrative Manual.
In November 2006, with the assistance of funding through the Federal Stafford Act, a small twenty-five (25) bed state-of-the-art hospital was constructed after the impact of Hurricane Rita. Through design and construction of the new hospital facilities, the following mitigating factors were incorporated into the District’s replacement hospital facilities, which included: (i) rebuilding the hospital ten (10) feet above sea level, and over six (6) feet above the ground with open flow-through piling foundation designed to mitigate velocity floodwaters, (ii) designing construction of interior wall finishes that were replaceable and able to withstand corrosion or erosion in the event of another disaster, (iii) the structural ability to withstand 140 mile-per-hour winds, (iv) building the Hospital with high-grade structural concrete, (v) elevated electrical supply, generators, and transformers, and (vi) other mitigating construction techniques. Notwithstanding these mitigation efforts, the Hospital suffered significant wind and water damage during Hurricane Laura in August 2020. The Creole, LA facility is the only hospital and emergency room in Cameron Parish. The damage sustained has caused the facility to be temporarily unusable creating a barrier to access to essential health care services in lower Cameron Parish, in particular, the access to all-important emergency services in an area devastated by natural disaster.
Time is of the essence in designing plans and specifications for this temporary replacement hospital as it is a Critical Need Facility for the District. The District expects the selected respondent, to produce all work for the Project relating to the temporary replacement hospital within forty-five (45) days of selection and execution of an appropriate AIA contract. The temporary replacement hospital must meet all current code requirements for acute care hospital facilities in Louisiana, as governed by all Louisiana laws and regulations including those established by the Louisiana Department of Health and the Centers for Medicare and Medicaid Services. The design of the Project must also comply with Stafford Act, HUD, and CDBG requirements. While this RFQ sets forth the requirements that must be satisfied, the District encourages Respondents to be creative and innovative with respect to their proposals to meet the goals of the District.
In addition to the work relating to the temporary replacement hospital under this RFQ, the Respondent selected will be engaged to provide separate architectural design services under a separate AIA contract relating to the repairs to and mitigation planning for the District’s currently damaged hospital facilities when the district elects to proceed.
This RFQ is a competitive selection process designed to engage a firm to provide Professional Architectural Services for the Project. More specific requirements for this proposal are set forth below.
B. TEMPORARY REPLACEMENT HOSPITAL REQUIREMENTS:
The temporary replacement hospital is expected to be in place for a minimum of three (3) years, with the possibility of longer as necessary based upon repairs to the damaged facilities. The temporary replacement hospital must be operational on August 27, 2021. In submitting proposals, Respondents should consider that the temporary replacement hospital will need, at a minimum, the following:
A. Emergency Room with three (3) bays;
B. Ambulance ramp access to elevated temporary replacement hospital emergency
department;
C. Ten (10) acute care inpatient beds: note State law affords the District with the option to construct less than ten (10) acute care beds pursuant to La. R.S. § 40:2109(B)(7);
D. Laboratory;
E. Reception;
F. Diagnostics, including but not limited to X-ray and CT machines;
G. Cubicles for staff/employees;
H. Triage Room;
I. Storage;
J. Generator;
K. Sewage and water treatment (as necessary);
L. Utility connections;
M. Temporary replacement hospital shall be elevated and capable of handling storm-surge at least equivalent to that experienced by Hurricane Laura (currently projected at an elevation of 14-15 feet above ground) and hardened to handle winds of 175 mph;
N. Temporary replacement hospital shall also comply with all Federal, State, and local laws, including but not limited to Centers for Medicare and Medicaid Services’ requirements, hospital building code requirements, Louisiana Department of Health hospital regulatory requirements, Louisiana Fire Marshall requirements, Uniform Building Code, Uniform Plumbing Code, National Fire Protection Association NFPA 101 Life Safety Code, Federal, State and Parish Public Health Codes, Underwriters Laboratories, and the current Facility Guideline Institute, and all other requirements that may apply;
O. All other minimum necessary components for acute care hospitals to meet the Centers for Medicare and Medicaid Services Conditions of Participation including the provision of all essential services;
P. The temporary replacement hospital can be of modular design and construction;
Q. All design work, including preparation of bid plans and specifications, must be completed within forty-five (45) days from selection of the architect and execution of the AIA contract.
C. SERVICES:
The services to be provided will include, but may not be limited to:
1. Utilizing existing plans and specifications and various programming materials already available and conducting meetings and interviews with the District’s representatives and Hospital’s operator:
i. To develop bid plans and specifications for the temporary replacement hospital, to develop the general design of this Critical Need Facility, and to obtain all permits from authorities having jurisdiction over the Project;
i. Note that these services, except for securing permitting, must be completed within forty-five (45) days of selection hereunder.
ii. When the District elects to proceed, to develop bid plans and specifications for the repair of the existing hospital facilities, to develop the general design of the mitigation plan for the hospital and to obtain all permits from authorities having jurisdiction over the Project;
2. If the District decides to go forward with construction of the temporary replacement hospital or the repair of the hospital facilities, assisting the District with the development and administration of the construction bid package in conformance with applicable federal and state requirements and supervising the bid advertising, tabulation, and award process, including preparation of the advertisement for bids solicitation, pre-bid conferences, conducting the bid opening, issuing a recommendation for award, and issuing the notice to proceed;
3. Obtaining all required federal, state, and local agency approvals and permits from authorities having jurisdiction over the Project;
4. On an accelerated, urgent basis, obtaining all approvals for inspections from the Louisiana Department of Health and the Louisiana Fire Marshall;
5. Conducting the pre-construction conferences;
6. Reviewing and approving all contractor requests for payment and submitting approved requests to the District;
7. Developing mitigation plan while interfacing with FEMA, GOHSEP, District’s consultants, and any other funding source for the Project that may become available, including HUD/CDBG;
8. Providing reproducible plan drawings to the District upon project completion;
9. Consistent with AIA contract provisions, conducting interim and final inspections and providing oversight of the construction;
10. Submitting certified “As Built” drawings to the appropriate agencies and the District; and
11. Other tasks typically associated with such an engagement.
The selected firm must be effective in its ability to design the temporary replacement facility within a limited scope project budget with the maximum level of creativity and opportunity for future expansion with a focus on creating a state-of-the-art healing environment that integrates care and service with the District’s mission and values.
D. PROJECT LOCATION:
The work is for the hospital’s facilities located at 5360 W Creole Hwy, Cameron, Louisiana 70631.
E. PROJECT DESCRIPTION, TIME IS OF THE ESSENCE:
The District is proposing for the Project to include:
1. Architectural design services for bid plans and specifications of a temporary replacement hospital as a Critical Need Facility that sufficiently addresses the emergent need for access to health care and hospital services in Cameron Parish at the District’s hospital campus;
2. Architectural design services that are provided on an urgent basis, with all work for relating to the temporary replacement hospital component completed within forty-five (45) days of being selected for the work under this RFQ;
3. Upon receipt of a request from the District, architectural design services under separate AIA contract, that sufficiently addresses the Hurricane Laura related damages to the District’s facilities with the purpose for reintegration of hospital services in the facilities, and to develop a mitigation plan that hardens and strengthens the District’s facilities to protect against significant wind speeds and related damage and also that elevates the facilities to protect against flood water damage.

Respondents will be evaluated on the basis of the written materials submitted, the results of referenced interviews and according to the following factors:
Background and Experience (35 points)
Firms shall:
a. Describe proposer’s firm by providing its full legal name, date of establishment, type of entity and business expertise, brief history, current ownership structure and any recent or materially significant proposed change in ownership.
b. Describe any prior engagements in which proposer’s firm assisted a governmental entity with
design services for projects with governmental assistance or funding programs. Proposer should include all examples of work on projects similar in scope with the Project. Proposer should provide a list of ongoing or completed projects. Please include in your response the following:
• Hospital name;
• Affiliated health system (if applicable);
• Location (City, State);
• Not-for-profit or community hospital (if applicable);
• Year completed (nothing pre-dating 2000);
• Number of beds;
• Square footage;
• Design services provided.
c. Please provide an overview of your firm, to at least include your firm’s size (including the number of licensed architects and number of ACHA accredited architects in your healthcare studio), and office locations.
d. Describe any issue the characteristics of which would be uniquely relevant in evaluating the experience of proposer’s firm to handle the Project.
e. Provide current information on insurance coverages carried by proposer’s firm, including amounts.
Specialized Knowledge (35 points)
Firms shall provide profiles for the following:
a. Site planning projects for three to five community-based hospitals and/or healthcare campuses.
b. Experience in hospitals that have been constructed in the last five years or are in the process of
being constructed.
c. List all healthcare project experience your firm has completed over the last 5 years (or are in the process of completing) in Louisiana.
d. Provide a list of public healthcare projects you have delivered through a Construction Management At-Risk format.
e. Provide a list of all litigation your firm has been involved with over the past five (5) years.
f. Provide a minimum of three (3) references for similar hospital and hospital campus projects.
g. Provide a list of your experience with urgent care/primary care facilities attached to community-based hospitals.
h. Provide a list of projects completed utilizing state and/or federal financing guarantees. (USDA, Louisiana Capital Outlay, HUD, FHA, CDBG, other).
i. Provide whether the Firm is able to complete the work hereunder within the short time frame required, and provide the Firm’s experience in prior projects where time was of the essence.
Professional Qualifications (20 points)
Firms shall:
a. Identify which of your offices will be responsible for delivering work for the Project and the contact name(s) of the individual(s) who will lead and manage the Project. Include a brief history
of the individual and a resume of each individual who will be assigned to the project.
b. Elaborate on the qualifications of the team members who will have leadership roles in the Project. Proposers should provide a list of all potential sub-consultants such as Surveyors, Geotechnical, HVAC, Mechanical, Electrical, Civil Engineers, Landscape Architects, etc. envisioned for the project. Proposer (firm) and individual resumes are required.
c. Assuming that key design activities will begin in the current year, will the firm be able to manage the workload required of this Project, along with the other projects it is currently engaged?
d. Submit AIA Document B305 Architect’s Qualification Statement.
Approach (10 points)
Firms shall:
a. Clearly describe the unique approach, methodologies, knowledge, and capability to be employed in the performance of the services.
b. Present innovative concepts, approaches, and methodologies to provide the services.
c. Please identify and list unique issues, in bullet point format, that your firm is aware of, which may be necessary to consider when completing similar development projects (example: site infrastructure and how it relates to the proposed development).
d. Please provide a timeline of activities in conformance with your approach.

I. RFQ RESPONSE:
Firms shall submit one (1) original plus five (5) hard copies of their qualifications (“Qualifications Statement”), along with a single electronic copy via email, on or before 4:00 P.M., CST, Monday, October 26th, 2020 to the contacts below. The original hard copy shall be signed by an authorized representative of the firm. Incomplete submittals and/or submittals received after this deadline may be rejected without review.

The deadline for submitting questions and clarifications regarding this RFQ is Monday, October 19th, 2020 before 4:00 p.m. CST. All notices, clarifications, addenda, and response to questions submitted regarding this RFQ shall be issued no later than Wednesday, October 21st, 2020. It is the sole responsibility of the submitting firm to remain apprised of changes to this RFQ.

The best responses will take into account all aspects of this RFQ in its totality, without regard to what section any aspect falls within.
The District reserves the right to reject any and/or all qualifications. The District reserves the right to modify this request for qualifications in its discretion at any time, upon notice to all applicants.
